    Telerehabilitation Transforms Recovery: Elevating Outcomes for Lower Limb Amputees

    Background: Telerehabilitation provides a viable alternative for individuals facing barriers to in-person rehabilitation because of transportation issues and physical limitations. It enables remote monitoring and guidance, allowing patients to continue rehabilitation at home. Objective: This study aimed to assess the effectiveness of a telerehabilitation program in improving physical and psychological outcomes for lower limb amputees and to evaluate adherence to a home-based exercise program delivered via video. Study design: A randomized controlled trial. Methods: Sixty participants with lower limb amputations were randomized to an experimental group (telerehabilitation) or a control group (standard care). The intervention involved a 4-week supervised phase followed by a 4-week unsupervised phase. Primary outcomes were physical capacity, measured by the Six-Minute Walk Test (6MWT), and psychological status, assessed using the Hospital Anxiety and Depression Scale. Secondary outcomes included adherence rates and patient satisfaction. Results: The experimental group showed significant improvements in 6MWT distance and Hospital Anxiety and Depression Scale scores compared to the control group. The mean 6MWT distance increased by 20% from baseline to the end of the supervised phase (P < 0.01) and was maintained during the unsupervised phase. Anxiety and depression scores significantly decreased (P < 0.05). Adherence was higher during the supervised phase (85%) but dropped during the unsupervised phase (60%). Conclusions: Telerehabilitation significantly improved physical and psychological outcomes for lower limb amputees. High adherence during the supervised phase emphasizes the importance of clinician support, whereas strategies are needed to sustain long-term engagement during the unsupervised phase.

    1,2,4-Triazole Conjugates as HEGFR Inhibitors: Synthesis, Anticancer Evaluation, and in Silico Studies

    A series of novel 1,2,4-triazole-acetamide derivatives was synthesized and evaluated for anticancer and hEGFR inhibitory activity. The compounds were obtained via multistep synthesis and characterized by spectroscopic methods. Cytotoxicity was tested against PC-3, MCF-7, A549, and K562 cell lines. Compounds <bold>18</bold>, <bold>19</bold>, and especially <bold>24</bold> showed notable antiproliferative effects, with compound <bold>24</bold> exhibiting higher selectivity and potency than gefitinib. It also induced apoptosis and inhibited migration in A549 and PC-3 cells, while selectively promoting invasion in PC-3, suggesting EMT-related behavior. In vitro kinase assays revealed compound <bold>20</bold> as the most potent hEGFR inhibitor (IC50 = 43.8 +/- 1.3 nM). Molecular docking and 200 ns molecular dynamics simulations confirmed its stable interaction with EGFR, particularly involving Cys797. These findings highlight compounds <bold>20</bold> and <bold>24</bold> as promising candidates for further development as EGFR-targeted anticancer agents.

    The Relationship Between Spiritual Well-Being, Resilience, and Adherence Among Patients Receiving Hemodialysis Treatment in Türkiye

    This study examined the relationship between spiritual well-being, resilience, and adherence of hemodialysis patients and the factors affecting them. The data were collected from 182 hemodialysis patients receiving treatment in a dialysis center who met the inclusion criteria by purposive sampling method in Istanbul. The data were collected with the patient description questionnaire, which measures patient sociodemographic characteristics and characteristics related to the medical diagnosis, the Spiritual Well-Being Scale, the Brief Resilience Scale, and the End-Stage Renal Failure-Adherence Questionnaire. Gender, educational status, employment status, and mean age of patients were found to be correlated with psychological resilience. Marital status, employment status, cohabitants, and mean age of patients were found to be correlated with spiritual well-being. Gender, number of weekly dialysis sessions, and dialysis competencies were found to be correlated with hemodialysis patients' adherence to their treatment. Hemodialysis patients' adherence was positively correlated with both the faith subscale of spiritual well-being and psychological resilience. According to regression analysis, gender and resilience explained 12.8% of the total variance of adherence. This study determined that resilience is an essential factor in increasing the adherence of hemodialysis patients.

    In Vivo and In Silico Evaluation of the Effects of Parsley (Petroselinum Crispum L.) Extract on Small Intestinal Tissue in Scopolamine-Induced Cognitive Dysfunction Model

    The brain-small intestine connection has become important in neurodegenerative diseases in recent years. In particular, the discovery of the relationship between Alzheimer's disease (AD) and the small intestine and the examination of the effects of AD on this tissue are important in this respect. Our study aimed to understand the effects of the experimentally created AD model in rats on the small intestinal tissue and the protective effect of the extract prepared from parsley leaves (PE). The experimental animals were divided into four groups in the study; Control, Scopolamine (Scop), Scop + PE and Scop + Galantamine (GAL). Oxidative stress parameters and activities of some important enzymes were examined in small intestinal tissues taken as a result of the experimental protocol. Additionally, in silico studies were carried out for bioactive molecules found in parsley leaves using data obtained from in vivo enzyme activity results. It was found that parameters examined for the damaged group, Scop, were reversed by PE and GAL treatment. As a result of in silico studies, it was determined that oxypeucedanin and phylloquinone had higher binding affinity than rutin for acetylcholinesterase (AChE). It has been observed that oxidative damage in the small intestine due to AD can be treated by the PE.

    Safety and Efficacy of Colchicine in COVID-19 Treatment: Systematic Review and Meta-Analysis

    Background: Colchicine is an anti-inflammatory drug used for the treatment of gout and other autoinflammatory conditions. Several trials reported promising results of the efficacy of colchicine in Covid-19 due to its antiinflammatory properties. However, applying these results to clinical settings remains the subject of ongoing research. Methods: We performed a systematic review and meta-analysis on the efficacy and safety of colchicine in the treatment of Covid-19. PubMed, Cochrane Library, Web of Science, and Scopus were searched for relevant studies. The primary outcomes were 28-day mortality, invasive mechanical ventilation, non-invasive mechanical ventilation, and ICU admissions. The risk ratio was used to compare effectiveness between the two groups. Subgroup analysis was done for C-reactive protein and duration of hospitalization. Results: 17 randomized controlled trials with a total of 25478 patients were included. The overall Risk ratio didn't favor any of the two groups in terms of 28-day mortality (RR =1.03, 95 % CI [0.93:1.15], P = 0.58), noninvasive mechanical ventilation (RR = 0.81, 95 % CI [0.44:1.48], P = 0.49), ICU admission (RR = 0.89, 95 % CI [0.56:1.41], P = 0.62). The overall mean difference (MD) did not show statistical significance between both groups in terms of Creactive protein (CRP) (mg/dl) (MD = -1.21, 95 % CI [-2.42:0.01], P = 0.05), lactate dehydrogenase (LDH) (U/ L) (MD = 50.95, 95 % CI [-92.07: 193.98], Ferritin (ng/ml) (MD = 128.08, 95 % CI [51.97:204.18], P = 0.001), ICU length of stay (MD = -0.09, 95 % CI [-0.34:0.15], P = 0.45) and duration of hospitalization (MD = -0.41, 95 % CI [-1.56:0.73], P = 0.48). Conclusion: In Covid-19 treatment, colchicine didn't result in significant benefits in terms of clinical outcomes. More large-scale randomized clinical trials with standardized dosages and long-term follow-up are needed for further investigation into the colchicine effect.

    Dual-Phase Malicious User Detection Scheme for IM-OFDMA Systems Using IQ Imbalance

    Physical-layer security techniques have contributed to the achievement of various security objectives in an efficient and lightweight manner. Thus, these techniques have been widely considered for limited-resource networks such as Internet of Things networks. Among the different security objectives, malicious user detection by exploiting physical-layer parameters has demonstrated efficient performance. In this work, malicious user detection in the recently proposed index modulation-based orthogonal frequency division multiple access (IM-OFDMA) is addressed. The proposed malicious user detection scheme exploits the hardware impairments, especially the in-phase and quadrature imbalance parameters, for both legitimate and malicious users to design a dual-phase efficient detection scheme. The proposed scheme accounts for the special characteristics of IM-OFDMA transmission that are different from other multiple-access techniques. The performance of the proposed scheme was evaluated considering detection probability and false alarm probability performance metrics. Moreover, closed-form expressions of these metrics were derived for both phases and were validated by Monte Carlo simulation results under different configurations of IM-OFDMA systems.

    Turkish Translation and Cross-Cultural Adaptation of the Pelvic Floor Dysfunction-SENTINEL Screening Tool: A Methodological Study

    Introduction and Hypothesis To translate and perform cultural adaptation of the Pelvic Floor Dysfunction-SENTINEL (PFD-SENTINEL) screening tool into Turkish and to establish its reliability and validity for female athletes. Methods The study included 200 female athletes (mean age 22 +/- 4 years). The translation of the PFD-SENTINEL, which comprised the symptoms related to PFD and item sections related to general risk factors and sports-related risk factors, was performed in accordance with international recommendations. The severity of PFD symptoms was assessed with the Pelvic Floor Distress Inventory-20 (PFDI-20). Psychometric analyses consisted of assessing the following: internal consistency reliability using the Kuder-Richardson-20 (KR-20) coefficient, construct validity using confirmatory factor analysis, and criterion validity. Results The reliability was acceptable for the PFD-SENTINEL items assessed using the KR-20, at 0.581, indicating moderate internal consistency. All fit indices except standardized root mean square residual indicated a perfect fit for the final models. Criterion validity was supported by positive correlations between the PFD-SENTINEL symptom score and the PFDI-20 score (r = 0.724, p < 0.001) and between the PFD-SENTINEL item score and the PFIQ-20 scores (r = 0.334; p < 0.001). Conclusions The Turkish version of the PFD-SENTINEL is a reliable and valid instrument to screen for PFD in female athletes.
