    Ultrasound Measurements of Pelvic Floor and Diaphragm Muscles in Chronic Obstructive Pulmonary Disease Patients With Urinary Incontinence

    BackgroundThe precise manner in which morphological properties of pelvic floor muscles (PFMs) and diaphragm muscle in Chronic Obstructive Pulmonary Disease (COPD) patients with Urinary Incontinence (UI) are affected remains unclear.AimThis study aimed to compare the ultrasound measurements of PFMs and diaphragm muscle in COPD patients with and without UI.MethodsThirty COPD patients with UI [UI group, age: 61.00 (42.00-70.00) years, body mass index: 27.86 (20.20-54.69) kg/m2] and thirty COPD patients without UI [N-UI group, age: 64.00 (47.00-70.00) years, body mass index: 27.11 (20.30-35.94) kg/m2] were included. The PFMs contaction assessment and diaphragm muscle morphological properties were evaluated using the Logiq S7/Expert ultrasound device. The percentage of change in thickness of diaphragm muscle, known as the diaphragm thickening fraction index (DTFI), was also calculated.ResultsIt was found that the PFMs contraction (p = 0.018) and DTFI (p = 0.016) values were significantly lower in the UI group compared to the N-UI group. No significant differences were observed in the diaphragm thickness score during the inspiration (p = 0.973) and expiration (p = 0.233) between the groups.ConclusionsPatients with COPD and UI exhibited diminished the PFMs contaction severity and DTFI compared to those with COPD but no UI. It should be considered that UI comorbidity in COPD may negatively affect PFMs contaction severity and diaphragm muscle morphological properties.

    Effects of the HEP® (Homeostasis-Enrichment Approach in Preterm Infants with Increased Developmental Risk: A Randomized Controlled Study

    Background and objectives: This study investigated the effectiveness of the Homeostasis-Enrichment-Plasticity (HEP) Approach in preterm infants with increased developmental risk, compared to the Traditional Treatment (TT) intervention for physical and occupational therapy.<br /> Materials and methods: Twenty-nine preterm infants (adjusted age, 4-10 months) were randomly assigned to two groups: the HEP Approach group and the TT group. The Peabody Developmental Motor Scales-2 (PDMS-2), Test of Sensory Functions in Infants (TSFI), and Beck Anxiety Inventory (BAI) were administered pre-and post-intervention. The intervention was implemented weekly for 12 weeks.<br /> Results: The baseline characteristics of the infants were similar. At the end of the treatment, a significant time effect was observed in motor skills and sensory functions across both groups, with improvements in all PDMS-2 and TSFI subtests (p < 0.05). Significant time x group interactions showed greater improvements in the HEP Approach group compared to the TT group for Fine Motor Quotient: F = 10.818, p = 0.003; Gross Motor Quotient: F = 5.691, p = 0.024; and Total Motor Quotient: F = 21.109, p < 0.001. For TSFI, the HEP Approach group showed greater improvements in Adaptive Motor Functions (F = 13.794, p = 0.001), Visual-Tactile Integration (F = 7.410, p = 0.011), and Total score (F = 11.316, p = 0.002). No significant time*group interactions were found for Reactivity to Tactile Deep Pressure, Ocular Motor Control, and Reactivity to Vestibular Stimulation (p > 0.05). Parental anxiety, measured by BAI, decreased significantly in both groups (F = 8.72, p = 0.006), but no significant time x group interaction was found (p > 0.05), indicating similar reductions in both groups. Conclusion: The HEP Approach demonstrated superior outcomes compared to the TT intervention in improving motor skills and sensory functions in preterm infants, while both interventions reduced caregiver anxiety.

    A Randomized Trial of Cervical Stabilization Exercise Training via Telerehabilitation for Migraine

    Purpose: To compare the effects of cervical stabilization exercise training via telerehabilitation (CSET-T) in addition to standard treatment on pain, forward head posture, cervical mobility, muscle performance, functional status, sleep quality, and quality of life in individuals with migraine in comparison to the standard treatment alone. Methods: The control group (n = 20) received standard treatment alone (medication+recommendations). The stabilization group (n = 20) was given CSET-T in addition to standard treatment 3 days a week for 8 weeks. Pain characteristics were assessed by using a pain diary, whereas forward head posture and cervical mobility were measured using a goniometer, cervical muscle performance (CMP) by using a pressure biofeedback unit, functional status by using the Migraine Disability Assessment Scale (MIDAS), sleep quality by using the Jenkins Sleep Scale (JSS), and quality of life by using the Headache Impact Test-6 (HIT-6). Results: Given the group-time interaction, it was found that pain frequency, intensity, duration, MIDAS, JSS, and HIT-6 scores decreased more significantly in the stabilization group when compared to the control group (p < .05). Moreover, the craniovertebral angle, cervical range of motions, and CMP values increased significantly more in the stabilization group (p < .05). It was found that there was no statistically significant difference between the groups in terms of compliance with standard treatment (p = .665). Conclusions: The study revealed that CSET-T in addition to standard treatment is superior to standard treatment alone in reducing pain, improving forward head posture, cervical mobility, muscle performance, functional status, and quality of life in individuals with migraine. © 2025 Elsevier B.V., All rights reserved.

    Prevalence of Fecal Incontinence After Childbirth in Turkey: Systematic Review and Meta-Analysis

    BackgroundFecal incontinence (FI), defined as the reduced or lost ability to control the elimination of solid or liquid stool and gas due to anal sphincter dysfunction, is mostly caused by childbirth.AimThis study aimed to determine the prevalence of postpartum fecal incontinence in Turkey.MethodsThis systematic review and meta-analysis were conducted between December 1 and December 31, 2024, considering inclusion and exclusion criteria. The literature search was performed using PubMed, Science Direct, Medline, Ovid, Ebsco CINAHL Plus, and Cochrane Library databases. The methodological quality of the included studies was assessed using the checklist developed by the Joanna Briggs Institute.ResultsA total of six studies were included in this review. The combined results indicated that the prevalence of postpartum fecal incontinence in Turkey is approximately 3%. Women who delivered by cesarean section had significantly lower rates of FI than those who delivered vaginally (z = 3.10, p = 0.002, CI: 0.39 [0.21, 0.71]). Additionally, the combined results of the studies showed no significant relationship between fecal incontinence and menopausal status, indicating that premenopausal and postmenopausal women had similar rates of fecal incontinence (z = 0.02, p = 0.98, CI: 0.97 [0.10, 9.47]).ConclusionFecal incontinence can occur after childbirth. The postpartum fecal incontinence rate in Turkey is 3%, and it is associated with the mode of delivery but not with menopausal status.     Glyphosate and AMPA-Induced Apoptosis and Epigenetic Alterations in HepG2 Cells: Upregulation of P53-BAX-CASP Pathways

    Glyphosate and its metabolite aminomethylphosphonic acid (AMPA) are environmental contaminants with potential toxic effects. This study aimed to investigate apoptosis and epigenetic alterations induced by glyphosate and AMPA in HepG2 cells. The IC50 values for glyphosate and AMPA were 6.19 mM and 8.13 mM, respectively, following 24 h exposure; mechanistic assays were conducted at sub-cytotoxic concentrations (50-500 mu M). Annexin V/PI flow cytometry revealed that AMPA significantly increased early apoptosis (up to 116 %, p < 0.001), while glyphosate elevated late apoptosis (up to 145 %, p < 0.001). Gene expression analysis showed significant upregulation of p53 (>= 1.49-fold), BAX (>= 1.82-fold), CASP3 (>= 1.37-fold), and CASP9 (>= 1.83-fold), with no significant change in BCL2. Epigenetic analysis indicated that both glyphosate and AMPA increased global DNA methylation, with fold changes ranging from 1.43 to 1.62-fold at concentrations of 100-250 mu M (p < 0.05). DNA methyltransferase genes DNMT1 (>= 2.44-fold), DNMT3A (>= 1.65-fold), and DNMT3B (>= 1.65-fold) were upregulated. Additionally, histone modification profiling showed elevated levels of H3K27me3, H3K9me3, and H3K9ac (p < 0.05), alongside increased expression of G9a (>= 1.64-fold), EZH2 (>= 2.14-fold), SETD1B (>= 2.15-fold), HAT1 (>= 2.40-fold), and SIRT1 (>= 2.57-fold), and downregulation of SUV39H1 (>= 0.27-fold). These findings reveal the molecular mechanisms of glyphosate and AMPA toxicity, linking apoptosis to epigenetic alterations and enhancing understanding of their risks.

    Assessment of Cross-Cultural Adaptations and Patient-Reported Outcome Measures Relevant To Shoulder Disorders in Turkish: a Systematic Review Using the Cosmin Methodology

    Background There are many shoulder assessment outcome measures in the literature that have been studied for validity and reliability. However, there is no study examining the Turkish-adapted patient outcome measures (PROMs) on the shoulder according to the COnsensus-based Standards for the selection of health Measurement Instruments (COSMIN) checklist. In addition, there is a small number of studies that carry out this examination on the shoulder internationally. Determining the most appropriate questionnaires for clinical use will also be an important guide in patient evaluation by filling the gap in both literature and clinical aspects.Objective Our aim in this study is to identify the valid and reliable Turkish scales used to evaluate shoulder pain and disability, to reveal how compatible these scales are with the aspects of study quality and psychometric quality according to COSMIN criteria list.Methods A systematic search was performed in the following electronic databases: MED-LINE, Web of Science (WOS), EMBASE, SCOPUS, and ULAKBIM were searched from inception to September 2024.Results A total of 2890 articles were found in the databases. Four articles were included additionally without a search. Then, after eliminating the duplicates, 2231 articles were included based on the title and abstract search. 63 articles were included in full-text review. After the full-text review, according to our criteria, 29 articles were included in the systematic review. According to the assessment by the COSMIN checklist, 28 articles received the final classification of "inadequate". Only 1 article (UCLA-Tr) received the final classification of "adequate".Conclusion There is a wide range of Turkish PROMs available for the shoulder, with only one fully meeting COSMIN methodology criteria. However, this does not imply that other PROMs lack clinical utility. Future studies should focus on refining PROMs by incorporating MCID and PASS scores and prioritizing their responsiveness to enhance their clinical relevance. Additionally, variations in PROM performance across different patient populations highlight the need for context-specific evaluations to ensure their applicability in diverse clinical settings.Trial registration PROSPERO registration number: CRD42022298425.

    Collagen Peptides and Saccharomyces Boulardii Cncm I-745 Attenuate Acetic Acid-Induced Colitis in Rats by Modulating Inflammation and Barrier Permeability

    Ulcerative colitis (UC) is an inflammatory bowel disease characterized by recurrent episodes of inflammation and tissue damage, with limited treatment options. This study aimed to investigate the effects of collagen peptides and Saccharomyces boulardii on acetic acid (AA)-induced colitis. Thirty-six male Sprague-Dawley rats were randomly divided into the following four groups: normal control (NC), colitis control (CC), collagen peptide (CP; 0.6 g/kg/day), and S. boulardii (SB; 250 mg/day). Colitis was induced by an intrarectal administration of AA in all groups except NC, and treatments were administered daily for 7 days. The therapeutic effects were evaluated by assessing the disease activity index (DAI), colon mass index, macroscopic and microscopic tissue damage, histopathological changes, zonula occludens (ZO)-1 protein expression, and myeloperoxidase (MPO) activity. The results showed that CP and SB treatments substantially alleviated DAI scores (p < 0.05) and reduced the colon mass index. Colon macroscopic and microscopic damages improved compared to the CC group (p < 0.01). Histologically, both treatments reduced inflammatory cell infiltration, crypt damage, and ulceration, with CP showing a slightly more pronounced effect. Immunohistochemical analysis revealed significant restoration of ZO-1 protein expression in the treated groups, indicating improvement in intestinal barrier integrity (p < 0.01). Furthermore, MPO activity was reduced in both CP and SB groups, significantly in the SB group (p < 0.01). These findings are consistent with previous studies that highlight the anti-inflammatory and barrier-enhancing effects of collagen peptides and probiotics in UC models.

    Prevalence of Colistin-Resistant Klebsiella Pneumoniae Isolates in Turkey Over a 20-Year Period: a Systematic Review and Meta-Analysis

    Klebsiella pneumoniae is one of the leading causes of healthcare-associated infections and poses challenges in its treatment owing to its high antibiotic resistance. The development of resistance to colistin, which is used as a last resort, has become a major public health problem worldwide. This study was planned according to the PRISMA guidelines and included studies reporting the prevalence of colistin-resistant K. pneumoniae in Turkey between 2004 and 2024 through a systematic literature review. A total of 28 original research articles were included in the meta-analysis. Data were analyzed using the SPSS and CMA software. The pooled colistin resistance of a total of 8916 K. pneumoniae strains from 28 studies included in this meta-analysis was found to be 1.63% (95% CI: 1.51–3.12). Colistin resistance increased significantly over time. A higher resistance rate was detected in the strains tested using the EUCAST guidelines and broth microdilution method. The year of the study and validation methods contributed to the heterogeneity observed in the studies. This meta-analysis reveals that colistin-resistant K. pneumoniae strains have increased over time in Turkey. Current data show that colistin resistance is not only a laboratory finding but has become a crisis, requiring urgent action in terms of hospital infection management and patient safety. Regional and global measures should be taken to ensure the appropriate use of antibiotics to control the development of resistance. © 2025 by the authors.

    Structural Equation Analysis of the Role of Appearance-Related Self-Conscious Emotions on Flow in Athletes: the Mediating Role of Autonomous Motivation

    The current study aimed to investigate the mediating role of autonomous motivation in the relationship between appearance-related self-conscious emotions (shame, guilt, authentic pride, and hubristic pride) and flow in athletes. The sample consisted of 217 athletes (57.6% male) aged between 18 and 39 years old (Mage = 21.77, SD = 4.53). The cross-sectional data were analyzed using structural equation modelling with bootstrapping method. Regarding direct effects, autonomous motivation was negatively correlated with shame and hubristic pride and positively correlated with authentic pride. Flow was positively related to autonomous motivation and hubristic pride. Regarding indirect effects, flow was negatively predicted by shame and hubristic pride and positively predicted by authentic pride through autonomous motivation. These results highlight the importance of autonomous motivation in the relationship between self-conscious emotions and flow in athletes. In particular, shame and authentic pride predicted flow fully through autonomous motivation. In conclusion, autonomous motivation mediates the relationship between self-conscious emotions and flow in athletes. We therefore speculate that improving autonomous motivation in sport psychology studies may help improve flow in athletes. © 2025 Informa UK Limited, trading as Taylor & Francis Group.

    The Effect of a Video-Based Covid-19 Paediatric Patient Education on State Anxiety in Children With Suspected Covid-19 Admitted To Hospital

    Background: Anxiety causes the long-term psychological impacts on children during the pandemic.Objective: The aim of this study is to reduce anxiety in paediatric patients and increase their adherence to treatment through a video-based COVID-19 paediatric patient education.Methods: This quasi-experimental study with pretest-posttest single group was carried out in a research hospital's COVID-19 Paediatric Emergency Area in Turkey between May and August 2020. The sample consisted of 128 children aged between 4 and 12 years.Results: In the study, it was found that 50.8% of the children who came to the COVID-19 Paediatric Emergency Area due to suspicion of COVID-19 were boys and their mean age was 8.54 & PLUSMN; 2.62. 21.1% of the children were hospitalised before and 73.4% were afraid of the hospital. State stress scores of the participants before the patient education were higher in those, who didn't attend school between ages of 4-10, than those who went to school. State stress mean scores of the children from all age groups decreased significantly after the education.Conclusions: The video-based paediatric patient education decreases the children's hospital-related anxiety during the pandemic and increased their adherence to treatment.
