We revised current health care developing approaches under the light of experience obtained throughout the pandemic so far. The aim is to be prepared in advance for possible upcoming pandemics. As in Covid 19 pandemics, such long lasting and widely affecting situations, durability is also very important together with flexibility and quickness. To provide durability, we need global policies taking the health to its center as well as health system policies.Key words: Covid-19, health system, emergency, crisis, vulnerability, durabilityArticle Citation - WoS: 2Citation - Scopus: 1Multidrug Resistance in Pathogens of Community-Acquired Urinary Tract Infections in Turkey: a Multicentre Prospective Observational Study(Tubitak Scientific & Technological Research Council Turkey, 2023) Sencan, Irfan; Karabay, Oguz; Altay, Fatma Aybala; Suzuk Yildiz, Serap; Simsek, Husniye; Gozukara, Melih Gaffar; Surme, SerkanBackground/aim: To have country-wide information about multidrug resistance (MDR) in isolates from community-acquired urinary tract infections (CAUTI) of Turkey, in terms of resistance rates and useful options.Materials and methods: We used a geocode standard, nomenclature of territorial units for statistics (NUTS), and a total of 1588 community-acquired isolates of 20 centres from 12 different NUTS regions between March 2019 and March 2020 were analysed. Results: Of the 1588 culture growths, 1269 (79. 9%) were Escherichia coli and 152 (9.6%) were Klebsiella spp. Male sex, advanced age, and having two or more risk factors showed a statistically significant relation with MDR existence (p < 0.001, p: 0.014, p < 0.001, respectively) that increasing number of risk factors or degree of advancing in age directly affects the number of antibiotic groups detected to have resistance by pathogens. In total, MDR isolates corresponded to 36.1% of our CAUTI samples; MDR existence was 35.7% in E. coli isolates and 57.2% in Klebsiella spp. isolates. Our results did not show an association between resistance or MDR occurrence rates and NUTS regions.Conclusion: The necessity of urine culture in outpatient clinics should be taken into consideration, at least after evaluating risk factors for antibacterial resistance individually. Community-acquired UTIs should be followed up time-and region-dependently. Antibiotic stewardship programmes should be more widely and effectively administrated.Article Citation - WoS: 3Citation - Scopus: 3Antioxidants, Enzyme Inhibitory Activities, and Phytochemical Profiles of Seven Medicinal Plants Grown With Organic Farming Techniques(Tubitak Scientific & Technological Research Council Turkey, 2023) Zor, Murat; Ozupek, Burcin; Pekacar, Sultan; Deliorman Orhan, DidemIn this study, the potential antihyperlipidemic, antidiabetic, and antiobesity effects of extracts obtained via infusion techniques from Artemisia absinthium L., Achillea millefolium L., Calendula officinalis L., Fumaria officinalis L., Mentha piperita L., Origanum onites L., and Urtica dioica L. grown with organic farming techniques were investigated using enzyme inhibition methods (alpha-glucosidase, pan-creatic cholesterol esterase, pancreatic lipase, and alpha-amylase). It is important that natural compounds or extracts to be used in the treat-ment of metabolic problems have antioxidant effects. The antioxidant effects of these species were tested in terms of ABTS and DPPH radical scavenging activity, ferric reducing power, and metal chelating capacity. Extracts (2 mg/mL) of M. piperita (91.43 +/- 0.90%) and O. onites (70.18 +/- 2.02%) showed potent inhibitory effects on the alpha-glucosidase enzyme. Among all species, only O. onites extract ex-erted an inhibitory effect on the cholesterol esterase enzyme close to that of reference compound simvastatin. While all extracts were effective in the tested antioxidant activity methods, it was determined that the M. piperita and O. onites extracts displayed particularly significantly strong activities in terms of ferric reducing power, ABTS radical scavenging effect, and metal chelating capacity. The total flavonoid and phenolic contents of all extracts were determined. The compositions of the most active extracts were analyzed for pheno-lic acids and flavonoids using RP-HPLC. The results of RP-HPLC analysis showed that the levels of ellagic acid (0.923 +/- 0.000 g/100 g extract) in the M. piperita extract and rosmarinic acid (0.813 +/- 0.003 g/100 g extract) in the O. onites extract were high. As a result, all species grown with organic farming techniques except U. dioica were found to have high antioxidant effects. In light of these findings, it was concluded that M. piperita and O. onites grown with organic farming techniques showed strong inhibitory effects in terms of the tested activities, especially for alpha-glucosidase and pancreatic cholesterol esterase, and that in vivo studies and activity-guided isolation studies should be conducted on these species in the future.Article Comparison of Intraoperative Biliary Anastomosis Stenting Technique in Living-Donor Liver Transplantation: Review of 41 Patients(Turkiye Klinikleri, 2022) Dönmez, R.; Balas, Ş.; Göktuğ, U.U.; Emek, E.; Tokat, Y.Background/aim: Biliary fistula is one of the most important complications in liver transplantation. Complications can vary from simple local peritonitis to death, and various techniques have been described to prevent them. In this study, we compared two different stenting methods used in biliary tract anastomosis in living-donor liver transplantation. Material and methods: We retrospectively analyzed data from 41 living-donor liver transplantations that were performed due to end-stage liver failure between August 2019 and November 2020. Patients were grouped according to the stenting technique used in biliary anastomosis. Postoperative biliary tract complications were investigated. Results: Biliary fistulas were observed in 2 (7.4%) patients in the internal stent group, while 4 (28.5) fistulas were observed in the external stent group. Biliary tract stricture was observed in 2 (7.4%) patients in the internal stent group, but there was no statistical difference in complications. The preoperative MELD score (p = 0.038*) was found to be statistically significant in regard to developing complications. Conclusion: Our study did not show the effect of stenting methods used during biliary anastomosis on the development of complications. However, larger randomized controlled studies are needed. © TÜBİTAK.Article Design and Implementation of a Low Cost and Portable Tactile Stimulator Br(Tubitak Scientific & Technological Research Council Turkey, 2022) Kazma, Coskun; Levent, Vecdi Emre; Cardak, Merve; Aydin, NizamettinWhen central nervous system has a problem, somatic area I and II respond to stimulation differently. Therefore, it is possible to identify some of the central nervous diseases when somatosensory on the fingertip is stimulated and responses are recorded and analyzed. We designed a system to stimulate the mechanoreceptors on fingertips. It is composed of a mechanical system for fingertip stimulation, an embedded controller, a control computer, and a software to control overall operation. During test, mechanoreceptors are stimulated according to the test protocols. Individuals' answers are recorded to be evaluated by the developed software. In this study, several design approaches for embedded controller were also examined and an FPGA based controller with the same functionality but higher performance was implemented. A test procedure was performed on 51 individuals at the department of pediatric psychiatry in a hospital, to investigate whether the sex has an effect on attention deficit and hyperactivity disorder.
