Master Thesis Kolcaba'nın Konfor Kuramına Göre Temellendirilmiş Hemşirelik Eğitiminin Anjiyografi Sonrası Yoğun Bakımda Hastaların Konfor Düzeyine Etkisinin Değerlendirilmesi(2025) Çakın, Zelal; Akıncı, Naile; Hemşirelik BölümüBu teze konu olan alan araştırması, randomize kontrollü bağımsız iki grup arasında prospektif olarak yapılmıştır. Kolcaba'nın konfor kuramına göre temellendirilmiş hemşirelik eğitiminin anjiyografi sonrası yoğun bakımda hastaların konfor düzeylerine etkisinin değerlendirilmesi amaçlanmıştır. Etik kurul ve kurum onayının ardından 01.04.2023-31.07.2023 tarihleri arasında İstanbul Kartal Koşuyolu Yüksek İhtisas Eğitim ve Araştırma Hastanesi'nin yoğun bakım ünitesinde koroner anjiyografi yapılan tüm hastalar çalışmanın evrenini, örneklemini ise dahil etme kriterlerini karşılayan 30 kontrol 30 girişim grubunda olmak üzere 60 hasta oluşturmuştur. Örneklem büyüklüğü 'G. Power-3.1.9.2' ile belirlenmiştir. Veriler Hasta Tanılama Formu ve Genel Konfor Ölçeği Kısa Formu ile toplanmıştır. Girişim grubundaki hastalara koroner anjiyografi öncesinde Genel Konfor Ölçeği Kısa Formu uygulanarak işlem süreci hakkında eğitim verilmiştir. İşlem sonrası ölçek tekrar uygulanmıştır. Kontrol grubunda ise Genel Konfor Ölçeği Kısa Formu, işlem öncesinde ve sonrasında uygulanmıştır. Girişim grubunda verilen eğitim, literatür taranarak ve uzman görüşü alınarak oluşturulmuştur. Her iki grubun ön-test son-test puanlarının karşılaştırılmasında Wilcoxon İşaretli Sıralı Testi ve Mann Whitney U Testi kullanılmıştır. Genel Konfor Ölçeği Kısa Formu uygulanması sonrasında, girişim ve kontrol grubundaki hastaların almış oldukları puanlar arasında istatistiksel açıdan anlamlı fark bulunmuştur (z:-1,40 - p:,047). Genel puan ortalaması, girişim grubunda daha yüksek çıkmıştır. Araştırmanın sonucuna göre katılımcılara uygulanan anjiyografi öncesi eğitimin, anjiyografi sonrası yoğun bakımdaki konfor düzeyini arttırdığı tespit edilmiştir. Hemşirelerin eğitici rolleri göz önüne alınarak koroner anjiyografi öncesi hastalara eğitim verilmesi önerilmektedir. How transplanted patients perceive their illness and their body can affect their physical and psychological recovery.Aim:This qualitative study was designed to evaluate self-esteem and body image in kidney transplant patients.Methods:The study involved 16 female renal transplanted patients. Data were collected using a two-part interview form developed by the investigators in accordance with the literature. We used Lundman's method to transcribe the interviews and coded the data from the interviews. The results of the study were discussed using the themes. The first three interviews were entered into MAXQDA, and then, additional interviews were analyzed.Results:Six main themes and 15 subthemes were generated based on the thematic analysis of the interviews with participants about body image and self-esteem. Themes included body acceptance, changes in body image, deterioration of social relationships, changes in self-esteem, spousal concerns, and organ perceptions. This study collected detailed data on body image, self-esteem, social relationships, and spouse/partner relationships after kidney transplantation.Conclusion:The change in participants' body image and self-esteem after kidney transplantation caused having negative feelings regarding sexual life, relationships, and social life and feeling unfamiliar with the transplanted organ.Article Citation - WoS: 9Assessment of the Effectiveness of Patient Education and Interviews in Improving Medication Adherence of Renal Transplant Recipients(Springernature, 2022) Aksit, Naile; Ozbas, Ayfer; Akinci, Serkan; Hemşirelik BölümüBackground Non-compliance with immunosuppressive drugs has been reported as the most significant cause of graft loss. Since non-compliance with immunosuppressive drugs is preventable, certain approaches based on the risk factors and causes of non-compliance can help eliminate this problem. Aims The purpose of this study is to assess the effectiveness of patient education and interviews in improving medication adherence of renal-transplant recipients. Materials and methods This study was designed as a randomized controlled trial. Using the G*Power program, the sample size was calculated as 60 subjects, with 30 in both the intervention group and control group. Data collection tools included a patient information form, a pretest-posttest, a drug monitoring form for kidney transplant patients, the Immunosuppressive Therapy Adherence Scale, measurement of tacrolimus blood levels, and a training booklet. Results The mean knowledge score in the intervention group was 12.17 +/- 3.39 at baseline, and it increased to 20.73 +/- 1.57 after the intervention. The mean scores on the Immunosuppressant Therapy Adherence Scale were 11.67 +/- 0.55 and 10.70 +/- 0.99 in the intervention group and control group, respectively. There was a statistically significant difference between the pre- test and post-test means on the Immunosuppressant Therapy Adherence Scale in the intervention group. The mean Immunosuppressant Therapy Adherence Scale score was higher in the intervention group. In the measurement of tacrolimus blood levels, medication adherence was found the be higher in the intervention group. The difference between the groups was statistically significant. There was a positive correlation between the tacrolimus blood levels and the Immunosuppressant Therapy Adherence Scale scores in both groups. Conclusions To conclude, our results have demonstrated that patient education and interviews improve immunosuppressant medication adherence in renal transplant recipients. Using direct or indirect methods proved similar outcomes, suggesting that both evaluation methods are safe.Article Effect of Progressive Muscle Relaxation on Postoperative Pain in Laparoscopic Living Kidney Donors: A Randomized Controlled Trial(BMC, 2025) Akinci, Naile; Hemşirelik BölümüAim This study aimed to evaluate the effect of progressive muscle relaxation on postoperative pain in laparoscopic living kidney donors. Design This was a randomized controlled, single-blind trial. This study was conducted in accordance with the CONSORT statement. Methods This study was conducted between January and September 2023 and included 62 patients (study group = 31, control group = 31) who met the inclusion criteria and underwent laparoscopic living donor nephrectomy in the transplant service of a private hospital in Istanbul province. A power analysis was performed to calculate the sample size and the power of the study (G*Power 3.1). Study data were collected using a patient information form, a postoperative patient follow-up and evaluation form to record the total number of bolus requests and administrations via of boluses with the PCA device and additional analgesic needs, and the Visual Analog Scale (VAS). The data obtained in the study were analyzed on the SPSS 22.0 statistical software. Results Additional analgesic requirements did not differ significantly between groups where progressive muscle relaxation exercises were applied and the VAS score and bolus requirement were significantly lower than in the control group (p < 0.05). Anxiety levels were also significantly reduced in the experimental group compared to both their pre-test scores and the control group (p < 0.05). Although the time to resume oral intake did not differ significantly between the groups (X-2 = 5.020; p = 0.170 > 0.05), the time to start mobilization was significantly earlier in the experimental group (X-2 = 8.808; p = 0.012 < 0.05). As a result of the study, it was found that progressive muscle relaxation exercises reduced postoperative pain, bolus requirements, and anxiety in patients undergoing living donor nephrectomy. Additionally, these exercises facilitated earlier mobilization following surgery.Article Böbrek Nakli Yapılan Hastalarda Sosyo-demografik ve Klinik Özelliklerin İmmünosupresif Uyumuna Etkisinin Değerlendirilmesi: Tek Merkezli Çalışma(2022) Akıncı, Serkan; Akıncı, Naile; Hemşirelik BölümüAmaç: Bu araştırmada böbrek nakli yapılan hastaların ilaç uyum oranlarını, sosyo-demografik ve klinik özelliklerin immünosupresif uyumuna etkisini değerlendirmek amaçlanmaktadır. \r\rMateryal ve Metot: Araştırma tek merkezli olarak kesitsel tanımlayıcı planlandı ve gerçekleştirildi. Araştırmaya organ nakli merkezinde son 6 yılda (2015-2021) böbrek nakli yapılan 210 hasta dahil edildi. Araştırmada veri toplama araçları olarak; “Hasta Bilgi Formu”, Böbrek Nakli Hastaları İçin İlaç İzlem Formu”, “İmmünosüpresif Tedaviye Uyum Ölçeği” kullanıldı. \r\rBulgular: Çalışmaya alınan hastaların İmmünosüpresif Tedaviye Uyum Ölçeği toplam puanları ortalama 11,16±0,82 olarak bulundu ve %57,9’unun immünosupresif ilaç kullanımına çok iyi uyum gösterdiği belirlendi. Sosyodemografik özelliklerden evli, üniversite mezunu, yüksek gelire sahip olanlarda İmmünosüpresif Tedaviye Uyum Ölçeği toplam puanları yüksek bulundu. Klinik özelliklerden ise bir günde kullanılan ilaç adedi arttıkça katılımcıların İmmünosüpresif Tedaviye Uyum Ölçeği puanlarının azaldığı saptandı. İlaç kullanımına yönelik eğitim alanlarda, immünosupresif ilaçların yan etkileri olmayan katılımcılarda İmmünosüpresif Tedaviye Uyum Ölçeği toplam puanları daha yüksek bulundu. \r\rSonuç: Böbrek nakli yapılan merkezlerde hastaların immünosupresif ilaç uyumlarının düzenli olarak kontrol edilmesi, uyumsuzluğa etki eden faktörlerin belirlenerek uyumu arttırıcı doğru yaklaşım yöntemlerinin geliştirilmesi, yaygınlaşması, süreklilik kazanması ve bunun hastane protokollerine eklenmesi gerektiğini düşünmekteyiz.Article Turkish Validity and Reliability Study of the Kidney Transplant Understanding Tool (K-TUT) in Kidney Transplant Recipients(Elsevier, 2025) Akinci, Naile; Genc, Kadriye Nilay; Toprak, Cagla; Hemşirelik BölümüAim: The study was conducted methodologically to evaluate the validity and reliability of the KTUT scale in kidney transplant recipients. Material and Method: The population of the study consisted of kidney transplant patients at a private hospital group in Istanbul, while the sample comprised patients who met the inclusion criteria and volunteered for the study. Based on ten times the number of scale items, 220 individuals were included in the sample to prevent data loss and to ensure high representativeness. In the study, data collection tools such as the "Patient Information Form" and the "KTUT" scale were employed. The test-retest study was repeated two weeks later with 50 participants. The participants in the retest were not included in other tests of the scale. Findings: The overall content validity index of the scale (S-CVI/Ave) was found to be 0.93. According to the experts' opinions, the content validity of the scale is high, and the majority of the items have been deemed appropriate for the measurement purpose. The internal consistency reliability coefficient of the scale (Cronbach's Alpha) was calculated as 0.789. This threshold indicates that the scale possesses an acceptable level of reliability for use at the group level. In addition, the results of the study reveal that the scores of the K-TUT scale show a significant difference in the 27 % lower and 27 % upper groups. Conclusion: As a result of the study, the K-TUT scale has been shown to reliably distinguish knowledge levels and to be an effective measurement tool in the field of kidney transplant. It is thought that using the Turkish version of the K-TUT to evaluate the knowledge levels of kidney transplant recipients will contribute to evaluation of developed programs and the improvement of patients' treatment processes.Article Evaluation of Hopelessness, Decision Regret and Desire for Retransplantation in Patients With Graft Loss After Kidney Transplantation(Elsevier, 2025) Akinci, Naile; Toprak, Cagla; Hemşirelik BölümüAim: In this descriptive study, it was aimed to evaluate hopelessness, decision regret, and desire to be transplanted again in patients who developed graft loss after kidney transplantation. Material and Method: The study sample consisted of all patients who developed graft loss after kidney transplantation between January 2017 and February 2021 in a private hospital in XXX, who met the inclusion criteria and volunteered for the study. "Patient Information Form", 'Beck Hopelessness Scale' and 'Decision Regret Scale' were used as data collection tools in the study. Findings: It was determined that participants experienced a moderate level of hopelessness after graft rejection (13.370 +/- 4.379) while the average scores on the decision regret scale were generally high (66.435 +/- 18.213). Regarding the desire to be transplanted again, it was observed that the majority of the participants (52.2 %) did not want to be transplanted again. According to the results of the correlation analysis, there was a significant correlation between hopelessness and decision regret scores (p* < 0.05) while there was no significant correlation with the desire to be transplanted again (p > 0.05). In addition, hopelessness (p = 0.034) scores of individuals who were not employed were significantly higher than those who were employed, and both hopelessness (p = 0.000) and decision regret (p = 0.048) scores of those without religious belief were significantly higher than those with religious belief. Conclusion: The findings of the study emphasize the critical importance of monitoring and psychological support processes before and after kidney transplantation. Understanding the emotional processes individuals experience after kidney transplant graft loss is important for the development of psychological support and intervention strategies.Article Citation - WoS: 1Evaluation of Depression, Anxiety, Stress, and Decision Regret in Kidney Transplant Recipients Depression, Anxiety, Stress, and Decision Regret in Kidney Transplantation(Bayrakol Medical Publisher, 2023) Akinci, Serkan; Akinci, Naile; Hemşirelik BölümüAim: This study aimed to evaluate decision regret, depression, anxiety, and stress scores in patients after kidney transplantation as well as the effects of patient characteristics on these variables. Material and Methods: This descriptive study enrolled 340 individuals who underwent kidney transplantation in a private hospital in Istanbul between January 2017 and February 2021. The study sample comprised 302 individuals who volunteered to participate in the study and met the inclusion criteria. The Patient Information Form, Depression, Anxiety, and Stress Scale (DASS 21), and Decision Regret Scale were used as data collection tools. Results: Mean depression, anxiety, stress, and decision regret scores were 2.454 +/- 3.427, 2.589 +/- 2.881, 1.825 +/- 2.073, and 18.311 +/- 20.123, respectively. Notably, these scores increased with an increase in age, and they were higher in single individuals, unemployed patients, nonbelievers, and those with chronic renal failure for a longer duration. Furthermore, depression, anxiety, and stress scores increased with increasing time after transplantation. In the present study, depression, stress, anxiety, and decision regret scores were significantly higher in patients who received transplants from their children. Discussion: The results of this study indicate that certain personal and clinical characteristics of kidney transplant recipients may affect depression, anxiety, stress, and decision regret after translation. Increasing the existing knowledge of such patients can minimize the risk of adverse effects of transplantation, including both somatic and psychological effects.
