We aimed to quantitatively determine the entry site and define the angular orientation of the ideal femoral tunnel with its relevance to the femoral valgus angle (FVA) and the femoral anteversion angle (FAA) to facilitate the creation of an ideal femoral tunnel during the LT reconstruction surgery. A total of 60 randomly selected CT images were obtained to reconstruct three-dimensional femur models. A virtual reamer representing the ideal femoral tunnel was placed in the femur models. The femur length, FVA, FAA, the femoral tunnel anterior angle, the femoral tunnel superior angle and the skin- and bony-entry sites were measured. The femoral tunnel angular orientation was strongly correlated with the FVA and the FAA. Mathematical formulas were defined by which entry site of the reamer and the anterior and superior angulation of the femoral tunnel could be estimated before the surgery. The mean skin-entry site was 67.3 mm distal and 0.1 mm anterior to the centre of the greater trochanter's superior border. The angular orientation of the femoral tunnel using FVA and FAA can be easily estimated using mathematical formulas before LT reconstruction surgery. The entry site and angular orientation of the femoral tunnel described in this study can be used to reduce dependency on the usage of fluoroscopy and the workload on the surgeon during the LT reconstruction surgery.Article Evaluation of Renal Functions of Inpatients With Mental Disorders(Slack inc, 2024) Dikec, Mehmet; Dikec, Gul; Ata, Elvan Emine; Ozer, Duygu; Dіkeç, Mehmet; Dіkeç, GülThe current study aimed to investigate the renal functions of inpatients with mental disorders. Data for this retrospective and descriptive study were collected from January 2021 to April 2021 from the records of patients who were hospitalized in the psychiatry clinic of a training and research hospital between 2018 and 2020. The study sample comprised hospital records of 376 patients. A significant negative relationship was determined between patients'glomerular filtration rate (GFR) and glucose level, duration of mental disorder, number of hospitalizations, and duration of medication use (p < 0.05). According to the analysis of patients' renal functions, mean GFR was statistically significantly lower in women with physical chronic diseases and diagnosed with personality disorders. Living donor liver transplantation (LDLT) has become an increasingly common surgical option because the number of cadaveric donors is insufficient to fulfill the organ needs of patients facing end-stage cirrhosis. Many centers are investigating different surgical techniques to achieve lower complication rates. We aimed to examine our complication rates in light of demographic data, graft data, and perioperative findings as a single-center experience. Methods. The study included one hundred and three patients who underwent LDLT for endvein, and bile anastomosis type rates; anhepatic phase; cold ischemia time; operation time; and blood product transfusion rates were analyzed. Biliary complications in patients with single or multiple biliary anastomoses, right or left liver transplants, and with or without hepatic artery thrombosis were analyzed statistically. Results. There was no significant difference in biliary complications between patients who underwent single or multiple bile anastomosis (P = .231) or patients receiving right lobe and left lobe transplants (P = .315). Although there was no statistically significant difference in the rate of portal vein thrombosis between the regular and reconstructed portal vein anastomosis groups (P = .693), the postoperative portal vein thrombosis rate was statistically higher in patients with left lobe transplants (P = .044). Conclusions. This study aims to investigate the protocol registration status and reporting of systematic reviews and meta-analyses published in psychiatric nursing journals. This descriptive study's data were obtained by scanning the 10 mental health and psychiatric nursing journals in which the studies of psychiatric nurses were most frequently published and by examining the systematic reviews and meta-analyses published between 2012-2022. A total of 177 completed studies have been reviewed. It was determined that 18.6% of the examined systematic reviews and meta-analyses had a protocol registration. Almost all (96.9%) of the registered studies were registered with PROSPERO, and 72.7% were registered prospectively. It was determined that the registration status of the studies changed statistically according to the country where the studies' authors were located. When the published studies were examined, it was determined that approximately one out of every five studies were registered. With the prospective registration of systematic reviews, biases could be prevented, and evidence-based interventions can be made based on the knowledge obtained.Article Citation - WoS: 3Citation - Scopus: 3Effect of Home Care After Cardiac Surgery(Managed Care & Healthcare Communications Llc, 2023) Aksut, Rabia Saglam; Kilic, DilekOBJECTIVES: Home care that maintains continuity of health care services after cardiac surgery is considered a complementary component of clinical care. We estimated that effective home care given with a multidisciplinary approach would contribute to decreasing symptoms and readmissions to the hospital after cardiac surgery.STUDY DESIGN: This experimental study with a 6-week follow-up period and a 2-group repeated measures design with pretest, posttest, and interval tests was conducted in a public hospital in Turkey in 2016.METHODS: We identified the self-efficacy levels, symptoms, and readmissions to the hospital of 60 patients (30 in the experimental group and 30 in the control group) throughout the data collection process, and we estimated the effect of home care on self-efficacy, symptom management, and hospital readmissions by comparing the data of patients in the experimental and control groups. Each patient in the experimental group received 7 home visits along with 24/7 telephone counseling services throughout the first 6 weeks after discharge, and patients were provided physical care, training, and counseling services during these home visits with the collaboration of their physician.RESULTS: Home care resulted in better self-efficacy and fewer symptoms in the experimental group (P < .05), and it also reduced readmissions to the hospital in the experimental group (23.3%) compared with the control group (46.7%).CONCLUSIONS: This study's findings suggest that home care, with a focus on continuity of care, decreases symptoms and readmissions to the hospital and improves the self-efficacy of patients after cardiac surgery.Article Effects of Peroperative Cold Ischemia Time and Anhepatic Phase in Adult Living Donor Liver Transplant Recipients: Operation Time That Is Not Affected by the Anhepatic Phase but Is Prolonged by Cold Ischemia Time(Elsevier Science inc, 2024) Anilir, Ender; Topcu, Feyza Sonmez; Sahin, Emrah; Oral, Alihan; Civan, Hasret Ayyildiz; Poyrazoglu, Kursat Orhan; Unal, Bulent; Sönmez Topçu, Feyza; Orhan Poyrazoğlu, Kürşat; Ayyildiz Civan, HasretObjective. It was aimed to examine the overall role of cold ischemia time and anhepatic phase durations in terms of peroperative blood transfusion needs, hospital stay conditions and postoperative charges, and survival in recipients. Material and Methods. One hundred forty-eight adult living donor liver transplant recipients (18 years and older) were included in the study. Whether the anhepatic phase and cold ischemia duration have an effect on the rates of surgery time, blood product transfusion, total hospital and intensive care unit stay, postoperative biliary complications, hepatic vein thrombosis, portal vein thrombosis, early postoperative bleeding, sepsis, and primary graft dysfunction. Was analyzed statistically. In addition, the effect of the anhepatic phase and cold ischemia time on graft survival was statistically examined by creating an average of the patient follow-up period. Results. It was observed that the operation time increased statistically as the cold ischemia time increased (P P = .000). No statistically significant fi cant relationship was found between other fi nd- ings and cold ischemia time and anhepatic phase. Conclusion. We collected data from 77 third-trimester pregnant women using a personal information form, Family Planning Knowledge Questionnaire, and the Satisfaction with Family Planning Services Scale and compared the data using chi-square test, Fisher's exact chi-square test, t-test for independent and dependent groups and Mann-Whitney U analysis. The experimental group participants (n = 37) received two informative video visits for contraceptive counseling, while the women in the control group (n = 40) received routine face-to-face Family Planning Services at their primary health care center. Participants in the experimental group showed a greater increase in knowledge levels and higher satisfaction with Family Planning Services than those in the control group (p < 0.05). In conclusion, we determined that telemedicine for contraceptive counseling is a promising option even beyond the pandemic.Article Citation - WoS: 1Citation - Scopus: 1Reliability of the Icd-11 Personality Disorder Severity Ratings and Diagnosis(Wiley, 2024) Aydin-Seyrek, Tugba; Gandur, Tarik; Turgut, Neslihan; Kunt, Duygu Aslan; Dereboy, FerhanThe present study aimed to investigate the interrater reliability of the dichotomous and dimensional personality disorder (PD) diagnoses based on the overall severity assessment on a rating form consisting of 18 anchored items encompassing diagnostic requirements of the International Classification of Diseases 11th Revision (ICD-11). We also aimed to examine the extent of consistency within the diagnostic requirements grouped under the domains of self- and interpersonal functioning, specific manifestations of personality dysfunction, and distress and impairment in psychosocial functioning. Our data involved a total of 184 inter-ratings of 46 consenting patients by the same set of four clinicians. The chance-corrected agreement levels were estimated at intraclass correlation coefficient (ICC) = 0.89 for the overall severity composite, ICC = 0.83 for the dimensional PD diagnosis and Fleiss' kappa = 0.77 for the dichotomous PD diagnosis. Internal consistency analysis of the overall severity composite and the domain composites revealed Cronbach's alpha coefficients approaching or exceeding 0.90 level. Our findings suggest that the diagnostic requirements listed in the ICD-11 and related documents for the severity determination in PD compose an internally consistent set. With the guidance of a rating form comprised of anchored items covering this set, competency-level clinicians are likely to perform reliable evaluations of the severity of personality disturbance, and dimensional and dichotomous PD diagnoses. The development of semi-structured interviews that would further facilitate the task of inspecting and rating each diagnostic requirement reliably will possibly enhance the implementation of the ICD-11 classification for PD around the world.
