Article Yüksek-Etkili Spor Atletlerinin Pelvik Taban Bilgisi, Farkındalığı ve Alt Üriner Sistem Semptomları: Kesitsel Bir Pilot Çalışma(2021) Celenay, Seyda Toprak; Düşgün, Elif Sena; Degirmendereli, Ahmet RasitAmaç: Pelvik taban ve alt üriner sistem semptomları (AÜSS) atletler için önemli konulardır. Bu çalışma, yüksek-etkili spor atletlerinin pelvik taban ile ilgili bilgi ve farkındalık düzeylerini ve AÜSS’lerini değerlendirmeyi ve cinsiyetler arasında pelvik taban bilgi ve farkındalık düzeylerini karşılaştırmayı amaçladı. Gereç ve Yöntemler: Toplam 88 atlet dâhil edildi. Pelvik taban bilgisi ve farkındalığı sorgulandı. AÜSS, Bristol Kadın Alt Üriner Sistem Semptom Anketi ve Uluslararası İnkontinans Konsültasyon Sorgulama Anketi-Erkek Alt Üriner Sistem Semptomları ile değerlendirildi. Pearson ki kare ve Fisher's exact testleri kullanıldı. Bulgular: Elli atlet (%56,8) futbolda, 21’i (%23,9) basketbolda ve 17’si (%19,3) voleybolda yer almaktaydı. Atletlerin çoğu pelvik taban kaslarını (PTK) duymamıştı (%73,9), PTK fonksiyonunu (%84,1) ve pelvik taban disfonksiyonu tedavisini (%86,4) bilmiyordu. Atletlerin çoğu pelvik taban kas egzersizlerini (PTKE) duymadığını (%84,1) ve hiç PTKE yapmadığını (%90,9) bildirdi. Kadın atletlerde pelvik taban bilgi düzeyi erkek atletlere göre daha yüksekti (p<0,05). Ayrıca kadın atletlerde en sık görülen AÜSS, depolama semptomlarına göre noktüri (%75,9) ve aciliyet hissi (%75,9), işeme semptomlarına göre duraksama (%62,1) iken; erkek sporcularda bu semptomlar depolama semptomlarına göre gündüz işeme sıklığı (%39,0) ve noktüri (%20,4), işeme semptomlarına göre tam boşaltamama (%11,9) idi. Sonuç: Atletler sınırlı pelvik taban bilgisi ve farkındalığına sahipti. Kadın atletler, erkek atletlerden daha fazla AÜSS’ye sahipti.Article Canlı Karaciğer Donör Adaylarında Hepatosteatozun Saptanmasında Biyopsi, Bilgisayarlı Tomografi ve Manyetik Rezonans Görüntülemenin Karşılaştırılması(2021) Sokmen, Bedriye Koyuncu; Şahin, Tolga; Koçak, Erdem; Oral, AlihanAmaç: Karaciğer donöründe hepatosteatoz (HS) varlığı, karaciğer transplantasyonu sonuçları üzerinde olumsuz etkilere sahiptir. Bu nedenle, donörde HS’nin tespiti, nakil öncesi dönemde hayati önem taşımaktadır. Bu çalışmanın amacı, canlı karaciğer donör adaylarında HS’nin saptanmasında karaciğer biyopsisi ve radyolojik yöntemlerin etkinliğini karşılaştırmaktır. Gereç ve Yöntem: Demiroğlu Bilim Üniversitesi’ne karaciğer transplantasyonu için donör adayı olarak kabul edilen 226 sağlıklı birey çalışmaya dahil edildi. Donörlerin demografik, histopatolojik, laboratuvar ve görüntüleme bulguları retrospektif olarak incelendi. Donörlerin bilgisayarlı tomografi (BT) ve manyetik rezonans görüntüleme (MRG) taramaları geriye dönük olarak yeniden değerlendirildi ve karaciğer yağ ölçümleri kaydedildi. Bulgular: Hastaların %39’u (88) kadın, %61’i (138) erkekti. Çalışma popülasyonunda ortalama yaş 34,3±8,7 yıl, ortalama ağırlık 78,0±12,6 kg, ortalama boy 169,1±9,6 cm ve ortalama vücut kitle indeksi 27,2±4,0 idi. Karaciğer biyopsisinde donörlerin %42’sinde <%5 HS vardı ve donörlerin %58’inde >%5 HS vardı. Hem BT hem de MRG, HS saptamada biyopsi ile anlamlı korelasyon gösterdi (p<0,05). Sonuç: Çalışmamızda MRG’nin BT kadar biyopsi ile ilişkili olduğu ve HS’nin saptanmasında rahatlıkla kullanılabileceği bulunmuştur. Karaciğer donörlerinde MRG kullanımı, iyonizan radyasyon içermemesinden dolayı nakil öncesi donör için daha uygun bir yöntem olabilir.Article Canlı Vericili Karaciğer Naklinde Retrospektif Donör Hepatektomi Sonuçları- Tek Merkezdeneyimi(2021) Andaçoğlu, Oya M.; Dönmez, RamazanAmaç: Donör hepatektomi tekniğimizi ve donör seçimimizi tanımlamayı, vaka serimizi gözden geçirmeyi, \rkomplikasyon oranlarımızı ve sonuçlarımızı bildirmeyi amaçladık. \rGereç ve Yöntemler: Yeditepe Üniversite Hastanesi'nde Ekim 2019- Kasım 2020 tarihleri arasında 41 ardışık \rdonör hepatektomi olgusunu retrospektif olarak inceledik. Komplikasyonlar Clavien-Dindo sınıflamasına göre \rderecelendirildi. Tüm olgular laparotomi ile yapıldı. \rBulgular: 41 donör olgusunun 38'i (%92,6) sağ lob, 2'si (%4,8) sol lob ve 1'i (%2,4) sol lateral segment donör \rhepatektomisi idi. Takip süresi 9 ± 2,2 aydı (4-16 ay). Sekiz (%19,5) komplikasyon vardı ve hepsi minördü \r(derece 1 veya 2). Derece 3 veya daha yüksek komplikasyon görülmedi. Üç donörde heterozigot Faktör 5 \rLeiden mutasyonu ve 4 tanesinde heterozigot protrombin mutasyonu vardı. Kalış süresi ortalama 6,4±1,4 gündü \r(aralık = 5-12). Kontrollü diyet ve egzersiz programı ile 10 donör kilo verdi. Bu hasta alt grubunda bir yara \rkomplikasyonu vardı. \rSonuç: Tek merkezli donör hepatektomi serimizi çok iyi sonuçlarla sunuyoruz. Ayrıca Vücut Kitle İndeksi > 30\rkg/m2\rolan donörler için başarılı kilo vermeyi de tanımlıyoruz. Donör güvenliği, canlı donör karaciğer naklinin \ren önemli bileşenidir. Donör sonuçları iyi oldukça, canlı vericili karaciğer nakli dünya çapında yaygınlaşmaya \rdevam edecektir.\rArticle Acute Serious Hepatitis of Unknown Cause in Children(Galenos Publishing House, 2022) Kılbaş, E.P.K.; Altındiş, M.On April 5, 2022, an increase in cases of acute hepatitis of unknown etiology was reported in previously healthy children under the age of 10 in the United Kingdom. Since there is no link between these patients, called acute non-HepA-E hepatitis, and viral hepatitis agents (hepatitis A, B, C, D, and E), the possible etiology, and pathogenesis of this emergency is being investigated. One of the alarming features of this epidemic is the high requirement for liver transplantation in a fraction of the cases. In cases other than hepatitis A, B, C, D, and E, a case definition is made by looking at a series of clinical pictures, including serum transaminase levels and age. As of August 26, 2022, 513 cases of acute hepatitis have been reported in Europe and 1,010 globally. Adenovirus was detected in 75% of cases tested in the UK, but data for other countries are still lacking. The role of other etiologic agents is still under investigation. The exact disease pathogenesis has not yet clear. Evidence of human-to-human transmission of the disease remains unclear. Epidemiological studies are critical in clarifying the uncertainties regarding the existence of links between the cases reported to date. Continuing the national and international surveillance activities of the countries in an organized manner is the most basic issue required for the elimination of the epidemic. To simulate the urolithiasis model 0.75% ethylene glycol (EG) and 1% ammonium chloride (AC) were applied to Sprague-Dawley rats. The rats were divided into four groups (n=8). The control group was given standard drinking water for 5 weeks. EG group received 0.75% EG in their drinking water containing 0.75% EG and 1% AC. PO extract (100 mg/kg) was given orally for 5 weeks to the preventive group and for last 2 weeks to the therapeutic group, respectively. At the end of experiment, 24-hour urine and kidney samples were obtained. In urine samples, calcium and citrate levels were decreased and oxalate level was increased in the EG group. In kidney samples myeloperoxidase, caspase-3, N-acetyl-β-glycosaminidase (NAG) activities, malondialdehyde (MDA), 8-hydroxy-2-deoxyguanosine (8-OHdG), tumor necrosis factor-α and interleukin-1β levels were increased while superoxide dismutase activity and glutathione levels were decreased in the EG group. These biochemical parameters returned to control levels in both PO treatment groups. Histological findings also correlate with these results. Our findings are suggested that PO treatments is effective in both preventive and therapeutic groups. © 2022 Marmara University Press.Article Comparison of Intraoperative Biliary Anastomosis Stenting Technique in Living-Donor Liver Transplantation: Review of 41 Patients(Turkiye Klinikleri, 2022) Dönmez, R.; Balas, Ş.; Göktuğ, U.U.; Emek, E.; Tokat, Y.Background/aim: Biliary fistula is one of the most important complications in liver transplantation. Complications can vary from simple local peritonitis to death, and various techniques have been described to prevent them. In this study, we compared two different stenting methods used in biliary tract anastomosis in living-donor liver transplantation. Material and methods: We retrospectively analyzed data from 41 living-donor liver transplantations that were performed due to end-stage liver failure between August 2019 and November 2020. Patients were grouped according to the stenting technique used in biliary anastomosis. Postoperative biliary tract complications were investigated. Results: Biliary fistulas were observed in 2 (7.4%) patients in the internal stent group, while 4 (28.5) fistulas were observed in the external stent group. Biliary tract stricture was observed in 2 (7.4%) patients in the internal stent group, but there was no statistical difference in complications. The preoperative MELD score (p = 0.038*) was found to be statistically significant in regard to developing complications. Conclusion: Our study did not show the effect of stenting methods used during biliary anastomosis on the development of complications. (MoC) ethanolic extract on ulcerative colitis (UC) and was explored in vitro and in vivo. Materials and Methods: The rats were divided into control (C), saline-treated colitis (AA), MoC extract-treated colitis (AA+MoC), and sulfasalazine (SS)-treated colitis (AA+SS) groups. Colitis was induced by acetic acid. MoC extract, SS or saline were given to the related groups for 3 days. Interleukine-1β, malondialdehyde, glutathione levels, myeloperoxidase activity, bax/bcl-2 ratio, caspase-9 and caspase-3 levels were measured in colon and macroscopic and histopathologic examinations were done. Total phenolic/flavonoid content and biological activity of MoC were evaluated by in vitro analysis. Results: Compared to the control group, with acetic acid application interleukin-1β levels, myeloperoxidase activity, malondialdehyde levels, bax/bcl-2 ratio, caspase-9 and caspase-3 levels were significantly upregulated, while glutathione levels were significantly decreased in the AA group. In contrast, MoC and SS treatments reduced interleukin-1β, malondialdehyde levels, myeloperoxidase activity, bax/bcl-2 ratio, and caspase-9 and caspase-3 levels. Glutathione levels increased upon MoC or SS treatment. Increased macroscopic and microscopic scoring with AA improved with MoC or SS treatment, but the MoC or SS treated groups had higher score values than the control. Also, in vitro results showed that MoC exhibited 2,2-diphenyl-1- picrylhydrazyl and 2,2'-azino-bis-3-ethylbenzothiazoline-6-sulfonic acid radical scavenging activity as well as significant antilipoxygenase activity. In addition, MoC extract showed a potent anti-inflammatory activity compared to standard indomethacin. Conclusion: Our biochemical, in vitro and histopathologic analysis indicate that MoC is likely to prove beneficial in UC therapy. © 2021 European Journal of Biology. For this purpose, male Sprague-Dawley rats were the control group (C), the elderly (A), the elderly with calorie restriction (ACR), the elderly who were exercised (AE) and the elderly who were exercised with calorie restriction (ACRE), they were divided into 5 groups. The control group was composed of three-month-old animals. The other group consisted of 15-month-old rats. Exercise and calorie restriction were applied for 6 weeks. At the end of the experiment, lipid peroxidation (LPO), nitric oxide (NO), glutathione (GSH) levels and superoxide dismutase (SOD), catalase (CAT), glutathione-S-trans-ferase (GST) and tissue factor (TF) were determined in brain and testicular tissues homogenates. As a result of the study, the A group’s brain and testis LPO, NO levels and TF activity increased, GSH levels and SOD, CAT and GST activities decreased, when compared to the C group. As a result of our study, an increase in oxidant damage was observed with TF activity in the brain and testis in the natural aging model, and positive effects of exercise and calorie restriction on the antioxidant levels in the brain were determined, especially in aging. © 2021, Istanbul University Press. It is thought that the side-effect mechanism is related to oxidant molecules derived from MTX metabolism. In this study, we examined whether the Petroselinum crispum extracts (PCr; parsley) of which the antioxidant properties have been previously shown, was protective against MTX induced liver damage. Materials and Methods: Sprague Dawley rats (female/male; 200-250 g) were used. MTX was injected intraperitoneally and PCr extract was given orally. A single dose of 20mg/kg MTX was administered to the groups that were to experience hepatotoxicity. Then, a physiological saline (MTX group) or PCr (2 g/kg, MTX + PCr group) treatment was applied for 5 days. The same treatments were applied to the other groups (control group, PCr group) for 5 days after a single dose saline injection. At the end of the study, the biochemical parameters were examined in the blood and liver tissues taken from animals sacrificed by decapitation. Results: MTX caused a significant increase in malondialdehyde and collagen levels and myeloperoxidase and caspase-3 activities, while glutathione levels were found to have decreased. PCr treatment showed protective efficacy by preventing these increases. Conclusion: It appears that the administration of PCr to MTX treated rats prevented the accumulation of lipid peroxides, inflamatory reactions and depletion of antioxidant glutathione, and thus protected liver tissues against oxidative stress. © 2021 European Journal of Biology. In our study, the amounts of steroid hormones were investigated in the sediment samples taken from the Marmara Sea. According to other studies, it has been determined that the excess of the hormone load in the sediments may be an indicator of human/animal sourced pollution, as well as the negative effects of the hormones mixed in the seas with the ecological cycle on the health of humans and animals.Methods: In our study, 31 selected human/animal, plant, natural and synthetic hormone-steroids were studied using Liquid Chromatography Electrospray Ionization Tandem Mass Spectrometry (LC-ESI/MS-MS). Methanol and QuEChERS were used as extraction methods. Sediment samples were taken from a total of 27 points selected for sampling at the Marmara Sea.Results: According to the results we found, the androgens: androsterone (24.50-1718.18 ng g-1), testosterone (86.30-1600.32 ng g-1); the estrogens: mestranol (33.73-228.32 ng g-1), equilin (53.44-1232.53 ng g-1); the progestagens; pregnenolone (37.50374.76 ng g-1), progesterone (39.96-405.60 ng g-1); levonorgestrel (325.25 and 937.93 ng g-1); the fecal sterols: cholestanone (57.57-1726.32 ng g-1), coprostanol + epicoprostanol (51.43-1370.33 ng g-1); and the plant sterol; campesterol (35.30-1859.90 ng g-1) were the compounds detected.Conclusion: Estrogens and progestogens are active components of birth control pills, and cholestanone and coprostanol + epicoprostanol are steroids that are indicative of human/animal pollution. Coprostanol + epicoprostanol and cholestanone, which are indicators of fecal pollution, were detected in all sediment samples. In our study, steroid hormones were detected for the first time in Sea of Marmara sediments and possible environmental risks were evaluated.
